Required Forms

All forms listed below must be completed and presented with membership fees prior to the start of class (to reserve placement) or on the first day of class.  Fees may be paid by cash, check, or credit card at the Point.  For insurance and liability purposes, participation is not permitted under any circumstances without required forms.  Pro-rated fees and refunds are available on a case by case basis, as determined by Club Manager and Board President.
